Police Background Check
All coaches, assistant coaches, managers,trainers and on ice helpers must have a police background check completed every season by October 31st.
1. Download and complete the following form.
2. Photocopy 2 pieces of ID and attach with staple to the completed form.
3. Put the completed form and photocopies in the Executive Mailbox in the First Aid Room at the Carp Arena addressed to: Risk & Safety
